You must:

Be at least 18 years old
Not be requesting the change to avoid creditors or criminal prosecution
Not be a registered sex offender or have recent felony convictions without a pardon

What You’ll Need to Provide

A completed Name Change Questionnaire
A copy of your valid Texas driver’s license or photo ID
A completed DPS fingerprint card (instructions provided)
